It’s easy to redeem your rewards points for great prizes, play games, enter our contests, and more! Simply click on our Smile Rewards Card, enter your 16-digit membership number (username) on the back of your card and your 4-digit password to enter into the fun world of our Patient Rewards Hub!
Returning this completed dental certificate to us will ensure that points are added to your Smile Rewards card. You can earn points for each regular cleaning and even more points for no cavities!
